How to Prepare for an Unannounced AAAHC Survey
The Shift to Unannounced Surveys
AAAHC has increasingly moved toward unannounced surveys as part of its accreditation process. While scheduled surveys give facilities time to prepare, unannounced surveys assess day-to-day compliance. This shift means that ASCs must maintain survey readiness at all times rather than preparing in the weeks before a scheduled visit.
For pharmacy services, this is particularly impactful. Medication management deficiencies are among the most common findings in ASC surveys, and they are often the result of lapses in daily routines rather than fundamental program failures.
Year-Round Readiness Framework
Daily Compliance Activities
These should happen every day the facility is operational:
- Temperature logs: Record temperatures for all medication storage areas (room temp, refrigerator, freezer) at the start of the day
- Crash cart seal check: Verify tamper-evident seal integrity and record in the log
- MH cart seal check: If applicable, verify seal and document
- Controlled substance perpetual inventory updates: Ensure all transactions from the previous day are recorded
- Expired medication check: A quick visual scan of medication storage areas for any approaching or past-due expirations
Weekly Activities
- 72-hour CS count review: Verify that all 72-hour counts were completed on schedule (Texas-specific under TSBP 291.76)
- Review of any open discrepancies: Ensure all CS discrepancies from the week have been investigated and resolved
- Medication storage area inspection: Check organization, labeling, and cleanliness of medication storage
Monthly Activities
- Full crash cart inventory: Open the cart, verify every item against the contents list, check all expiration dates
- Full MH cart inventory: Same as crash cart
- Expiration date audit: Comprehensive check of all medications in the facility
- P&P review: Ensure policies are current and any recently updated policies have been disseminated to staff
Quarterly Activities
- Consultant pharmacist visit: Review all pharmacy operations, documentation, and compliance metrics
- Quality improvement data review: Analyze medication error reports, near-misses, and compliance metrics
- Staff competency verification: Ensure all staff handling medications have current competencies documented
Annual Activities
- Complete P&P review and update: All pharmacy policies and procedures reviewed, updated, and re-approved by medical staff
- Formulary review: Annual formulary review with medical staff approval
- DEA registration verification: Confirm registration is current and renewal is scheduled if approaching expiration
- Biennial inventory planning: Ensure the DEA biennial inventory is on schedule
Pharmacy-Specific Survey Tips
What Surveyors Look For First
Experienced ASC administrators and consultant pharmacists report that surveyors typically begin with:
- The medication storage area: Is it clean, organized, and properly temperature-controlled? Are medications within expiration dates? Are controlled substances properly secured?
- Crash cart documentation: Are daily checks documented? When was the last full inventory? Are all medications within date?
- Controlled substance records: Is the perpetual inventory current? Do the counts match? Is waste properly documented?
- The consultant pharmacist agreement: Is it current? When was the last visit? Are visit reports on file?
Red Flags That Trigger Deeper Review
- A single expired medication can trigger a facility-wide medication audit
- Gaps in temperature logs suggest the monitoring program is not consistently followed
- CS count discrepancies without documented investigations suggest inadequate oversight
- Staff who cannot describe basic medication safety procedures indicate training gaps
Staff Preparedness
Every staff member who handles medications should be able to answer:
- Where are the medication policies and procedures?
- What is the procedure for a medication error?
- How do you report an adverse drug event?
- Where is the crash cart and when was it last checked?
- Who is the consultant pharmacist and when was the last visit?
Creating a Survey-Ready Culture
The most successful ASCs treat compliance as a continuous process, not an event:
- Assign a compliance champion: One individual responsible for tracking all pharmacy compliance activities
- Use checklists and tracking tools: Daily, weekly, and monthly checklists ensure nothing is missed
- Conduct internal mock surveys: At least annually, preferably quarterly
- Make compliance visible: Post compliance dashboards, track metrics, celebrate sustained compliance
- Address findings immediately: When a gap is identified, fix it the same day and document the corrective action
